Physical Information

We now also have the option of submitting physical forms electronically through FamilyID as well.  After entering the student’s name, you will have the ability to scan your signed physical form on-line.  Physicals must be completed on the approved PIAA CIPPE form which is available on the link below.  Please remember that physicals must be dated after May 1st to be eligible to play.  ALL SPORTS PHYSICALS SHOULD BE SCANNED INTO FAMILY ID OR TURNED IN TO THE HIGH SCHOOL ATHLETIC OFFICE.  THE SCHOOL NURSES WILL NOT BE ACCEPTING SPORTS PHYSICALS.

If your child is injured, they will need to have a doctor sign off on the PIAA section 9 recertification form which is linked below before being able to return to play:
